After 6 months of use: multi-state Medicaid Chronic Care Management add-on demonstrates consistent high engagement & patient & provider satisfaction

MotiSpark expanded it’s Chronic Care Management add-on service with Precise Telehealth after pilot phase with dialysis patients. MotiSpark expanded to add 5 more Chronic Care Management nurses and their patients in four states. Predominantly Medicaid patients who qualified for CCM services with 2 or more chronic diseases with highest percent being hypertension and diabetes. Majority of patients feel into 2 categories: African Americans in Illinois and Hispanics in Texas. In total: 46 people in Texas, 26 people in Illinois, 32 people in Indiana and a couple in Idaho.

MotiSpark was not integrated into EHR and all data was de-identified.

  • Very high patient engagement & satisfaction: patients watched each video 1.7x on average and engaged with platform 12/month. Frequency of nudges depended on patient selections. 

  • CCM nurses reported that patients who received MotiSpark were happier, easier to work with and healthier

  • Incorrect enrollment data was the biggest hurdle encountered with ~20% of enrollments being unsuccessful.

  • Patients did not report any technical problems with platform, but there were requests for more specific kinds of MotiSparks (diet and exercise.)
